The whole mole was removed completely with minimal scar. There was no contracture observed. The left eye can be closed completely without deformation. Upper eyelids of both sides were symmetrical. Eyesight was not affected.

Aesthetic dermatology Case study – Mole removal
這個巨大的墨痣生於左眼上眼皮，病人見過整形外科醫生，建議以手術方式割除，之後再縫線。當中還涉及各種各樣的皮瓣手術及皮膚移植程序。手術的副作用就是會令上眼皮變小，導致左右眼皮不對稱，甚至有可能眼皮不能完全閉合導致眼角膜變乾及壞死。病人認為如此副作用不可以接受。我建議以分段式二氧化碳激光把之去除。
這個手術並非一次完成。其實是把一個手術，分開四次完成。每次手術分隔四至六個星期，待一個傷口完全復原後才開始下一個手術。
傷口越小，癒合速度越快，而且疤痕越輕微。
但是激光治療並非沒有任何風險，主診醫生必須準確掌握 CO₂ 激光在皮膚各層的穿透深度，以確保徹底清除黑色素細胞（即痣的根源），同時盡量減少疤痕的形成。

This large mole developed on the left upper eyelid since the patient was born . The patient had previously consulted a plastic surgeon, who recommended surgical excision followed by stitching. This involved different types of skin flaps and/or skin transplantation. However, the surgery could result in a smaller upper eyelid, leading to asymmetry between both eyelids. In more serious cases, the eyelid might not be able to close completely, causing the cornea to dry out and potentially become necrotic. The patient found these side effects unacceptable.
I recommended CO₂ laser removal/cauterisation in stages instead.
This procedure is not completed in a single session. It is essentially one surgery divided into four separate treatments. Each session is spaced four to six weeks apart to allow full healing of the wound before beginning the next.
Smaller wounds heal faster and result in lighter scarring.
But this is not without risks. The operating physician must accurately gauge the penetration depth of the CO₂ laser across different skin layers to ensure complete removal of melanocytes—the root of the mole—while minimizing the risk of excessive scarring.
