注射類固醇後3個月痊癒，一年多後都沒有番發。

討論：

患者頭部部份頭髮於幾個星期至幾個月內迅速脫落，脫髮的位置的頭皮看來沒有異樣。不痕不痛。

鬼剃頭的學名其實叫斑禿。成因是身體的免疫系統出錯，攻擊部份頭髮（以致脫落）引致。10至20％患者有家族史。兒童及年輕人較常見，但亦可發生在任何年齡的人身上。

it is a chronic organ-specific autoimmune disease, mediated by T cells affecting hair follicles and nails.

Associated with autoimmune thyroid disease.

Follicular damage occurs in anagen followed by rapid transformation to catagen and to telogen; then to dystrophic anagen status. While the disease is active, follicules unable to progress beyond early anagen and do not develop normal hair.

Look for ‘exclamation mark’ hairs.

分類：Classification:

Alopecia areata: 斑禿，部份頭髮脫落

AA totalis: 全頭頭髮完全脫落

AA universalis: 全身所有毛髮完全脫落

Ophiasis: 頭髮邊緣範圍脫落

Nails: Fine pitting (hammered brass) of dorsal nail plate. Mottled lunula, trachyonychia, onychomadesis.

Investigation: rule out SLE (ANA), rule out secondary syphilis (PRP test), rule out tinea capitis.

治療：

外用類固醇藥膏，如果無效，可以把類固醇注射入頭皮內。如果無效，加上病情嚴重，可以考慮口服類固醇，甚至口服CYCLOSPORIN。亦可嘗試PUVA治療。也有皮膚科醫生使用 Induction of allergic contact dermatitis.
